Senior Healthcare Recruiter at HealthCare Recruiters International Healthcare Recruiters seeks a full‑time Masters level Social Worker (LCSW preferred but will accept ASW/ACSW/AMFT) for our client’s 501(c)(3) nonprofit organization that provides housing and mental health support to individuals with intellectual and developmental disabilities. This job is HYBRID, meaning predominantly working on laptop and cell phone servicing individuals in Sonoma and Solano counties. Some office visits may be needed for occasional team meetings. Best if live in or close to those counties. NOT accepting out of state candidates as the work is within these California areas only. Job Duties & Responsibilities: Responsible for the entire case management process, including assessment and evaluation, planning, authorizing, advocacy, monitoring, reviewing documentation for completion. Responsible for providing a safe and welcoming environment for individuals, developing case plans according to the needs of each individual and providing ongoing support as they work toward achieving goals. Conduct initial and ongoing assessments and follow‑up visits to each individual. Assess individuals’ needs and link them with outside services such as primary medical care, outpatient treatment, government financial assistance etc. Maintain all documentation and contacts as required by law, regulation and policy. Advocate on behalf of the organization’s individuals to protect their rights under the law and ensure that they receive those services and benefits to which they are entitled. Collaborate with other departments and outside providers to support and advocate for the organization’s individuals. Respond to crises, including providing de‑escalation intervention and support to highly agitated individuals. Keep supervisor informed of the status of the caseload and unmet needs in the community. Ability to cover evening and weekend shifts with certain caseloads.

WORK ENVIRONMENT & SCHEDULE

Occasional travel between sites, community events, and meetings. Standard work hours are Monday through Friday, with some flexibility required for evening or weekend events. A hybrid or in‑office schedule may be available depending on organizational needs. Must be comfortable working in both office and community settings. Occasional lifting or physical assistance may be required during events or site visits. Must have a valid driver’s license, reliable transportation, and the ability to travel locally. Job Qualifications: Master’s degree in Counseling, Psychology, Social Work, Sociology from an accredited college or university. LCSW or ASW/ACSW/AMFT. 3+ years’ experience working with the DD/ID and dually diagnosed individuals. Experience in a non‑profit, case management, social services, and mental/behavioral health. Prior experience interacting and working with Regional Centers, FHAs or other services for people with developmental disabilities. Experience providing individual, group therapy, or other clinical services. Experience working with or leading multidisciplinary teams. Experience with therapeutic arts, such as art, music, or dance therapy. Knowledge of community resources and referral networks related to mental/behavioral health services. Proficient in crisis intervention techniques and risk assessment. Excellent communication and interpersonal skills for effective client engagement and collaboration with interdisciplinary teams. 100% paid for Medical, Dental, and Vision. 3% Company Match.
